Place ham bone (or ham hocks) in crock pot. Add water and broth. Add garlic, basil, and red pepper flakes.

Add chopped swiss chard next and pack down slightly. Top with tomatoes, chick peas and finally, chopped ham.

Turn slow cooker to lowest setting and cook for 8 to 10 hours. Serve with crusty bread and grated cheese (not included in nutrition profile)
